Here is a modern day fictional league that I set up in OOTP and I am not exactly sure how I could do the same thing in PureSim.
As of right now it is a two league (with two divisions in each league). Each division contains five teams, for a total of 20 teams. At some point I would love to add a division to each league for a total of 30 teams. The unique thing is that each division is state based (Washington, Oregon, California and Hawaii). So each team in a division is from that state. If divisions were add, I would be adding new states to the league. I have it currently set up for interleague play (and would like to keep it that way).
Neither league has a DH (I hate the DH, but that is for debate on another day). Right now I have it set up with four level of minor leagues (I realize it would have to drop to three if set up in PureSim).
How could I go about setting this up in PureSim? I am not sure if the city database would have five cities from each of the states mentioned (except maybe California) or any future state divisions that may be added later. How could I get an interleague schedule for the current twenty team league? Will the current version of PureSim allow for a long and enjoyable play through or will stats go off the rails after a few seasons (especially since I like to sim some history first before picking up and playing)?
Please let me know if this is possible and how you would go about setting it up. I appreciate the advice even if it is not possible.

Alright, let me see if I can take a crack at this...
First off, regarding your concern as to whether or not there are enough cities in the list to accommodate each state, fear not! You can select the same city for the other teams in the state, then once the team is created, go to the respective team's Front Office page and click on the [Edit Region/Finances] button along the top. You can then rename the Region Name and further down the City Name to fit whatever you wish to use. Thus, if a chosen state only has one city in the drop-down menu, select that same one for all the teams you wish to use, then manually adjust away the duplicates. Conversely, there's the [Create New Region] button that would let you ADD one!
Now, creating the league structure is somewhat limited; you must select from one of the pre-determined league formats that the game provides. From what you described, there is one that fits your needs ideally:
20 Teams 2 Leagues 2 x 5 team divisions per league
The DH concern is not an issue; you can turn the DH feature ON or OFF for each league in the HOME screen -> Options and Utilities link) -> Association Settings tab.
I do not believe that you can deliberately create Interleague Play as an option unless you pick the standard MLB 162-game schedule, but I may be wrong there.
